California divorce requires a stack of Judicial Council forms. Here’s every form you’ll need, explained in plain English.
Forms to Start the Divorce
| Form | Name | Purpose |
| FL-100 | Petition — Marriage/Domestic Partnership | The main document that starts your divorce |
| FL-110 | Summons — Family Law | Official notice to your spouse that you’ve filed |
| FL-115 | Proof of Service of Summons | Proves your spouse was served |
| MC-030 | Declaration of Venue | States why you’re filing in this county |
Response Forms
| Form | Name | Purpose |
| FL-120 | Response — Marriage/Domestic Partnership | Your spouse’s response to the petition |
| FL-115 | Proof of Service | Proves the response was served on you |
If your spouse doesn’t file a response within 30 days, you can proceed with a default judgment.
Financial Disclosure Forms (Required)
| Form | Name | Purpose |
| FL-140 | Declaration of Disclosure | Cover sheet — certifies you’re providing complete financial info |
| FL-142 | Schedule of Assets and Debts | Lists every asset and debt with values |
| FL-150 | Income and Expense Declaration | Your income, expenses, and tax information |
| FL-141 | Declaration Re Service of Final Disclosure | Certifies final disclosures were exchanged |
Both spouses must complete and exchange Preliminary Declarations of Disclosure. This is NOT optional — even in an uncontested divorce.
Child-Related Forms (If Applicable)
| Form | Name | Purpose |
| FL-105 | Declaration Under UCCJEA | Required for any case involving minor children |
| FL-311 | Child Custody and Visitation Attachment | Details of the custody arrangement |
| FL-342 | Child Support Information and Order Attachment | Child support details |
| FL-155 | Financial Statement (Simplified) | Simplified version of FL-150 |
Judgment Forms
| Form | Name | Purpose |
| FL-180 | Judgment — Dissolution/Legal Separation | The final divorce decree |
| FL-190 | Notice of Entry of Judgment | Notifies both parties the divorce is final |
| FL-170 | Declaration for Default or Uncontested Dissolution | Required for default judgments |
| FL-343 | Spousal/Partner Support Order Attachment | Details of spousal support |
The Marital Settlement Agreement
While there’s no specific Judicial Council form for the marital settlement agreement, this is arguably the most important document in your divorce. It’s a custom contract between you and your spouse covering: – Property division – Debt allocation – Spousal support – Child custody and visitation – Child support – Any other terms
How Many Forms Total?
A typical uncontested California divorce with children involves 15–25+ forms. Without children, it’s still 10–15+ forms. Each must be completed accurately — errors cause court rejections and delays.
